The Ranger medic (military occupational specialty 91B) provides advanced trauma management across the operational spectrum in which the 75th Ranger Regiment is employed. Ranger medic duties, both in combat environments and in training, medical training, professional progression, and medical assets in the Ranger battalion are detailed. Ranger medic training management tools and techniques are discussed and illustrated. The role of the combat lifesaver, force modernization, and interoperability issues facing the medical team are discussed. The Ranger medic is a capable special operations tactical medic. The Ranger medic (military occupational specialty [MOS] 91B) is the tactical medical provider for the 75th Ranger Regiment, the U.S. Armys elite light infantry. Through training, leadership, and experience, he becomes one of the most effective combat medics in the U.S. Army. He is capable of performing across the operational spectrum of 75th Ranger Regiment employment, providing advanced trauma management in the worst of conditions. This article is based on a presentation to the U.S. Army Special Operations Command (USASOC) Surgeons Conference in December 1998. Within Special Operations Forces (SOF), the Special Forces Medical Sergeant MOS 18D is the epitome of the SOF medic. The force structure, selection, training, retention, and employment issues are well developed for the 18D medic, because the nearly 700 18Ds form the bulk of SOF medics. The other SOF medics, which include the 91B Ranger medic, the 91B flight medics of the Task Force 160th Special Operations Aviation Regiment, U.S. Air Force flight medics, and Navy Sea, Air, Land independent duty corpsmen, do not have the same refined program. There are approximately 56 91Bs within the Ranger regiment, who represent a small fraction of SOF medics. This small number of Ranger medics provided an opportunity for the development of a Ranger medic training and career progression plan.

As I was completing my residency at Childrens National Medical Center, I, as many of my third-year colleagues, found myself wondering, where to go from here? Everything in our previous experience had been laid out in precise order, like check boxes waiting to be completed. Completing residency was the last check box on the list. We now had choices–fellowship, primary care, advocacy, etc. Enter Information Technology (IT). I can honestly say that, if I had been seriously considering a clinical job offer, I would probably be working in a clinic somewhere today. The lack of job, however, opened me to trying something new and different. When I heard about the informatics position, in which I would be able to help create a new computerized order entry system at Childrens, my hospital, I listened. and accepted the offer. Flash forward to Day One. My first day working in IT was definitely an experience to be remembered. I did not have to arrive at work until 8 am! I was able to sleep in! I actually had an office (cubicle) with my own phone and a laptop! As a resident, we had a lounge/classroom and had to vie for phone and computer usage wherever we went. In IT I had my own phone number and could give my new extension to whomever and could receive direct calls and messages.

Most folklorists would appreciate the basic premise of Michael Symons book-cooking is an artistic, social, and intellectual skill that has been overlooked and undervalued, both by scholars and by the world at large. Symons argues that contemporary western society tends to regard cooking and the wide range of activities connected to it as insignificant, trivial, and simply a means to an end that is bodily pleasure rather than spiritual edification, and therefore, unworthy of intellectual scrutiny. Similarly, individuals who cook are rarely accorded respect for the skills required to master their duties or for their role in society. This book is meant to remedy the situation by offering an apologetics for cooks and cooking. In the process, Symons attempts a theory of the world according to cooks. It is a colorful and intriguing theory, and, using Australia as his primary example, he regales the reader with anecdotes, quotations, and examples from literary, popular media, ethnographic, and historical sources. The book offers a selected compendium of famous thinkers on food and cooking. In that, it is very useful, and it is fascinating to read quotes and paraphrases from throughout history and across cultures on the subject of cooking. As would be expected with a work of this magnitude, however, it has a number of problems, namely, interpretations of theory and data, use of sources, and organization. To my mind, Symons discussions of theories relevant to cooking tend to be superficial. He gives rather summary dismissals of major thinkers, rarely placing them in cultural context or giving complete analyses of their theories. Plato, Barthes, Marx, Geertz, Douglas, Levi-Strauss, Bourdieu and others are all evaluated in reference to their appreciation of cooking, which is probably as good a criterion as any, but his readings of these theorists are too biased against them to be accurate or useful. It is also unclear whether these writers are being used as data, as proof of the validity of Symons arguments, or as interpretive frameworks. To his credit, he clearly and honestly states his own opinions. After discussing Mary Douglass structuralist approach to the grammar of a meal, he states: ” [T] his kind of pattern is certainly intriguing, but fails to speak to me as the deep-seated cultural logic that is claimed” (103). His assessment of Lévi-Strauss is more positive: “[He] puts familiar ideas into fancy language, but at least he takes cooking seriously” (105).

Life coaching is the new celebrity craze and, with Paul McKenna promoting it on TV, it doesnt look like stopping. Mitya Underwood meets three people whose job it is to help people lead a happy and successful life It was after taking part in a TV dating show that Rachael Mulholland decided to transform her life and take up life coaching full time. She was hoping to find Mr Right after winning BBC 3s The Bachelor programme two years ago.
There are around 3,000 “life coaches” in America, and numbers are rising. Even so, coaches in the States are still waiting for the day when people stop saying, “What is life-coaching?” and ask, “Who is your coach?” A coach is a hybrid of business advisor, mentor, therapist, counsellor - and grandma, who thinks youre perfect and wants you to be happy. Ive worked with counsellors for years, writing about their work. I admire it enormously, but whats exciting about coaching is the directness. A counsellor could have your problems and solutions taped in minutes, but months can pass as you are gentled through the process of discovering it for yourself. Coaches instead are straight-talking (but always positive), and will say what strikes them as the truth from the first session. Coaching is achievement- focused. It feels adult, and fast - you are encouraged to take action from day one to become more profitable, healthier, deal with unfinished emotional business, have more fun, make better relationships, or whatever is holding you back. Coaches are trained to handle anything but they tend to specialise in certain professions or life situations. It can be surprising. With my coach, Madeleine Homan, who specialises in creative types, I addressed my procrastination and writing blocks in our first talk. She didnt advise me about how to work harder, but asked me to take more time off - making one working day a week my “creative day” when I do whatever appeals. I tried out this and other suggestions, and just seven weeks later I work with more pleasure, less angst and increased effectiveness. I feel healthier, more energetic and more relaxed. Madeleine is in New York (not so crazy, as a half-hour call costs under pounds 4), but there are coaches in Britain (most working for corporates), 50 training with Coach University, the American “virtual” course, carried out through teleconferencing and e-mail. Some come from caring professions, but felt it made sense to look at practical and financial issues as well as the emotional. Some began as business consultants but couldnt ignore personal issues. They have been coaching for years, usually face-to-face, but some are starting to offer a telephone service. Sherrie Charlton is a UK coach. Vaughn Khelifa, who works in the pharmaceutical industry, was coached by her for 18 months.” I thought it would be like training - being lectured to by an expert. Instead Sherrie helped me think differently: what am I looking for from this; is this the best approach for that person? At the end, I ask myself if I handled it the best way, or how I could improve on it. It has given me confidence and skills that have helped me progress in my job and develop even better relationships with my husband and daughter.” Sherrie believes you have to look to your own development when helping others develop, and spends a quarter of her work hours on this. She is doing a Masters in personal and organisational development as well as a Coach University course. The training is fascinating in itself (Ive signed up). It is commonsense, with a spiritual slant, underpinned by a philosophy of personal excellence. Vocabulary can be puzzling (or a source of joy). One student supplied a word to a trainer who thanked her for “up-languaging me”; Madeleine thought I was confessing to promiscuity when I said I was a “slut”, though I was only admitting to cobwebs and clutter. On June 21, law enforcement personnel from around the nation gathered at the Mississippi Agriculture and Forestry Museum in Jackson to discuss a hot topic - heavy equipment theft. Sponsored by FBI-LEEDA and the National Equipment Register (NER), the summit drew personnel from 45-50 law enforcement agencies from as far away as Denver who wanted to learn more about the growing problem of heavy equipment theft and how to fight it. Joey Gonce is director of the agriculture and livestock theft bureau at the Mississippi Department of Agriculture (MDAC). His main focus at the MDAC is agriculture theft, but he has fielded calls from construction companies that are missing equipment, and as a retired highway patrolman, hes seen it all.

Firms are facing financial difficulties because they are too slow to grasp new technology, according to Graham Whitehead, principal consultant and futurologist at BT, who spoke at the ITService Management Forum conference last month in Brighton. Whitehead pointed to encyclopedia publisher Britannia as a great example of a firm using new technology and learning to adapt. The company has published a L39 DVD of its complete encyclopedia collection in addition to the hard copy volumes. “Firms are going out of business so quickly, Whitehead added. They have about 10 months to get it right once they start having difficulties.” Geoff McMullen, president of the British Computer Society, agreed that change was becoming increasingly rapid and was multi-dimensional. Half of what we know today will be out of date within three years, he said. But he cautioned firms not to get carried away by technology. You can get the new technology, but your infrastructure must be developed slowly and carefully. People must be trained and the vision communicated within the organisation, or the technology means little.”
TECHNOLOGY The bad news is that diffusion rates through any industry are difficult to measure, and monitoring the pace of invention is worthless in an industry as large and diverse as mortgage banking. We cannot answer a question of prevalence or permanence without guessing. To avoid getting dragged through the mud, firms are doing their own technological assessment prior to writing contracts for purchase of equipment or software. The good news is that the assumed results of currently available technology can be measured, and hence, the assessment can be financially sound. This months column presents a few ideas relevant to the acquisition of technology and the measure of its net worth. The effect of technological change is measured by the annual increase in output per employee hour of labor. Simply, this measure tells us how much more volume the new technology will produce using existing staff. Because many people in an organization are usually affected by a new system, it is important that gross changes be computed, using the average output per employee, rather than just the evaluation of one particular employee.

For Americans the celebration of Christmas is often considered one of the most important holidays of the year. Because of the diverse heritages and customs in addition to Kwanzaa a tradition begun in the later part of the twentieth century the American Christmas consists of traditions from not only the German but English Dutch and other Eastern European countries as well. Falling during the same month as the Jewish observance of Chanukah or Hanukkah (the Feast of Lights) and the African-American celebration of Kwanzaa the season of Christmas serves as a time of celebration feasting and a search for miracles.While Christmas generally is considered the celebration of Jesuss birth the early Puritans who settled the New England region refused to celebrate the occasion. Disagreeing with the early church fathers who established the holiday around a pagan celebration for easy remembrance by the poor the Puritans considered the observance secular in nature. Set during the winter solstice when days grow dark early Christmas coincides with the Roman holiday of Saturnalia; the date December 25 marks the celebration of Dies Natalis Invicti Solis or the birth day of the Unconquered Sun by the Romans. Puritans believed that these pagan customs which included no work feasting and gift giving were inappropriate for the celebration of the Lords birth. While the northern colonists did not observe the day the southern colonists celebrated in much the same style as their British counterparts–with banquets and family visits. Firecrackers or guns were used to welcome the Christ Child at midnight on Christmas. The residents of New York (New Amsterdam) celebrated the season in a similar fashion as their Dutch ancestors had with St. Nicholas Day (December 6) established to honor the patron saint of children. Gradually all observances became centered around the date December 25 with traditions becoming mixed and accepted between different ethnic backgrounds.

Existing buildings are often too low to allow the use of conventional/vertical layouts for charging systems, at least not without considerable civil investments. The day bins, dosing screws, scales, etc., for gravity flow arrangements all require height, and that can mean high costs. Pneumatic/horizontal weighing and dosing systems can offer a more reasonable solution, even for difficult materials. Conveying pneumatically from silos or buffer hoppers directly into a scale above the process machine reduces height requirements and increases flexibility. Possibilities of pneumatic/horizontal scale systems arc presented. If high weighing accuracy is to be achieved on any scale system, it requires an even and controlled volumetric feed for each component fed to the weigh hopper. Pneumatic scale systems are no different. Each material has a dedicated conveying line fed by rotary feeders especially designed and selected to ensure a very even product flow is achieved. Variable speed drives are then used to adjust the product flow according to the requirements of the scales. Coarse and fine dosing rates are used to achieve a repeatable and guaranteed dynamic accuracy, usually in the range of 0.2-0.3% of maximum scale value, for both pressure and vacuum systems. Like conventional systems, the accuracy depends on the number of components and the scale time available in the process cycle. In some circumstances, scale times may be so short, component quantities so many and/or accuracy requirements so high, that more than one scale has to be used. Alternatively, the use of a holding bin below the scale can effectively extend the weighing cycle by removing waiting time, allowing the next weigh cycle to start independent of product demand in the process. Today, it is possible to install systems with a distance of gt;100m between storage and scale, with batch weights of 200 kg and cycle times of 90 seconds.

By designing materials and structures at the molecular level, engineers believe they can create products featuring powerful chemical and mechanical properties hitherto unknown to the technical community. The current market for fuel cells is $218 million and will increase to $2.4 billion by 2004, reaching $7 billion by 2009. Most fuel cells today are found in distributed generation applications in the power market, providing dedicated electrical energy to hospitals, restaurants, schools and other facilities. In the next few years, fuel cells are expected to show up in tens of thousands of automobiles, and by 2010, automotive fuel cells are estimated to have nearly 4 percent of the consumer market - 608,000 vehicles.
Its payback time for business technology providers - 2003 produced a bumper crop of exciting new products and initiatives. Management identifies the technologies that will deliver the greatest productivity gains in 2004 and beyond. The hype has largely evaporated and technology is now focusing on pure productivity outcomes, helping organisations work smarter, so that ultimately we can all work less. Civil engineering construction continued to increase in the third quarter. The value of construction put in place in this segment of construction was 26.2% higher than a year ago. A year-on-year decrease was recorded only in the construction of residential buildings (see table). In interpreting the value of construction put in place, we note that these figures exclude the activity of smaller firms (see note below table), whose main activity is estimated to be residential construction. According to seasonally adjusted data, activity increased relative to the previous quarter only in civil engineering, whereas activity in residential construction was at its lowest point in the last two and a half years. The growth of employment in the construction sector continued. In the nine months to September, the number of employees surged by 11.0%. As in 2006, the highest increases in employment were recorded in general construction and building completion. The share of firms that were unable to find workers declined in November after Octobers record-high value. According to business trends in construction, 47% of enterprises (employing 52% of workers) reported “shortages of skilled workers” as a limiting factor in November. This figure is smaller than in the previous two months, but still well above the average value of this indicator in the last five years (since data have been available).
The growth of construction activity was the in 15 years. This robust growth was again driven by the construction of motorways, in addition to the pick-up in the construction of non-residential buildings, which reached its highest level since 1999 The value of construction put in place surged by 15.3% over 2005. Growth in civil engineering totalled 16.8%. The construction of buildings rose by 14.0%, within that, non-residential construction was up 17.7% while residential construction increased by 2.6%. Based on detailed data about the issued building permits we estimate that the construction of wholesale and retail trade buildings and hotels picked up last year. In interpreting the data on the value of residential construction put in place we note that these figures exclude the activity of smaller companies (see the note under the table), in which, in our estimate, the construction of buildings is the main activity. The number of workers in the construction sector rose sharply in 2006, especially in the final quarter. The number of people employed in construction was on average 7.5% higher in 2006 than in 2005. In the fourth quarter alone, it increased by as much as 9.7% over the year before The number of employees by activity rose particularly in building completion, in the latter pari of the year it also increased in general construction In lerms of employee status, the number of individual private entrepreneurs and their employees rose the most last year. The number of planned dwellings and the total planned floor area of buildings continued to increase in the final quarter of 2006. The planned floor area of new buildings and extensions was 27.5% higher than in the same quarter of 2005 (43 8% for non-residential buildings and 16.9% for residential buildings) In 2006 as a whole, the total floor area of the planned buildings increased by 35.8%. within that by 19.7% for residential buildings and by 56 0% for non-residential buildings The building permits issued last year were granted for the construction of 8.260 new dwellings, which is 19.6% more than in 2005 and 47.0% more than the average of the previous seven years (since these data have been available)

Burglars love them, but most householders ignore them until they get blocked by falling leaves. Autumn is the time to consider your drainpipes. According to Dyno-Rod research, 50 per cent of the population dont bother, while just 12 per cent do an annual clear-out. A Dyno-Rod survey last May found that property surveyors would like to see more potential house-buyers checking their drainpipes prior to purchase. Meanwhile, Treve Rossoman is appalled at the way builders have played fast and loose with plastic drainpipes since the war. “Frontages have been ruined by houses being converted into flats. If bathrooms are at the front, they smash a hole through that wall . . . its cheaper than taking the waste round to the side. Its an abomination.” By contrast, Modernist architects have gone to some lengths to hide drainpipes, thereby contravening their honesty-is-the-best-policy rule. Michael Johnson, an architect, is bemused. “Since the Beaubourg Centre in Paris, much architecture has been about expressing a buildings functions. Yet the Beaubourg codes all its external services pipework in bright primary colours, except the drainpipes.” In their first historical incarnation, drainpipes were dominating features. The medieval equivalents were lead-lined wood gutters plus magnificently carved gargoyles that spurted rainwater through stone monster mouths, safely away from church walls. It was the Georgians who started the drainpipe hokey cokey, in and out of sight, ordering them, with the 1724 Building Act, round the back of houses lest they ruin the clean look of facades. But even the Victorians, for all their table-leg-covering modesty, did not try to hide drainpipes completely. Now we have become coy. “Look at Stansted Airport,” says Michael Johnson. “There is no visible evidence of how the rainwater gets from the roof to the ground.” For some people, though, drainpipes are a point of pride. The Charles Brooking Collection at the University of Greenwich has more than 100 rainwater heads dating from between 1800 and 1960, including examples used by builders to date their work. For a huge variety of modern drainpipes, head for the Building Centre at Londons Covent Garden. “As far as I know,” says the information manager, Darren Jarvis, “this is the only place in Britain where you can come and look at an extensive collection of contemporary drainpipes. Even high- class builders merchants tend to stock only two or three plastic makes.” As well as handing out catalogues for copper, flower-blue and ruby-red drainpipes, the Building Centre offers advice. They can tell you about Guardian Security Pipes, “flush to wall and immovable” to flummox burglars, or, if you are more concerned with aesthetics, put you in touch with suppliers such as J JW Longbottom of Yorkshire, who stock cast-iron drainpipes. But cast iron doesnt come cheap. A job lot of four downpipes, 20 metres of half round guttering and four rainwater heads will cost around pounds 150 in plastic. Double that figure for basic cast-iron equivalents, and treble it if you want plenty of fancy bits such as turret-style hopper heads and fleur-de-lis earbands. Treve Rossoman decided his bank balance couldnt stand such an outlay. “My house is 1904, but someone put square plastic drainpipes on. I painted them dark green, which is, along with dark red and blue, a traditional colour for 19th-century guttering and downpipes.” Of course, the best way to appreciate the variety of drainpipes is simply to look around. My personal favourites are those at Londons Victoria and Albert Museum, where gargoyles overlook pedestrians. But as part of a Neo-Gothic building, the gargoyles are toothless, literally. Real estate practice is a broad and widely defined area of the law. In a pristine sense it deals with the esoteric subject of real property law and titles. In practice the subject matter handled by real property lawyers is much broader. Many of the areas of law within real estate practice are areas of specialty themselves. Real estate lawyers examine and evaluate real property titles, handle real estate sales, purchases and other transactions, foreclose mortgages, represent developers and contractors in drafting and negotiating development and construction agreements, handle real estate financing, structure and draft the governing documents for planned developments such as condominiums, serve as counsel to community and condominium associations, draft and negotiate leases, litigate title, boundary, and other land disputes, and advise clients respecting zoning and land: use matters. The examination of real estate titles requires knowledge of corporations and other forms of business organization, wills, trusts and estates, and a practitioner handling transactions must understand contractual, business, and tax subjects as well. The subjects that are tested on the certification exam attest to this diversity. They are title issues; real estate finance; environmental and land use; condominiums and homeowner associations; landlord tenant; closings; sellers, buyers, brokers, and title insurers; real property litigation; taxation of entities and other tax issues; and ethics. Some hold an impression that at least some aspects of the real estate practice are not challenging, that the practice is increasingly title company, form, and paralegal driven, and that when business is light any lawyer with a good paralegal can Ado real estate closings. An estimated 264,000 women opted to increase their breast size with breast implants in 2004, according to the American Society of Plastic Surgeons, a form of cosmetic surgery known as breast augmentation. Additionally, another 75,800 women had their breasts surgically lifted, a procedure known as a mastopexy. On the opposite end of the spectrum is breast reduction. This procedure, which may be considered a cosmetic surgical procedure, is often performed to relieve significant health and emotional problems resulting from overly large and/or heavy breasts. In 2004, the Society notes, 105,500 breast reductions were performed, a 25 percent increase over 2000. Whether you choose to have your breasts increased, lifted or reduced, its important that you understand not only the benefits of the procedures, but the risks, costs (health insurance rarely pays unless there is some medically necessary reason for the surgery), and various surgical options available to you.
Breast Augmentation The majority of breast augmentations are performed using breast implants, silicone spheres filled with saline or sterile salt water, or, more recently, silicone. Silicone-filled implants are thought to provide a more natural appearance and are less likely to deflate than saline implants. One advantage to saline implants, however, is that some implants allow the size to be adjusted after surgery by adding solution up to six months after the surgery.
In the early 1960s, it was shown that silicone was the most inert and least reactive of all implantable substances, and it seemed ideal for breast implants.(2) Since then, nearly three million women have had breast augmentation with implants made of silicone. In the early 1990s, there was speculation that silicone implants might be the cause of immune system disorders. There has been much litigation on this issue, however, research has shown these disorders to occur no more often in women with implants than in women without implants.(3) For most of the nearly 40-year history of breast augmentation with silicone implants, the incision locations have been in the inframammary crease, the axilla, and around the nipple-areola (Figure 1). All three approaches permit the placement of both saline-filled or silicone gel-filled implants in front of or behind the pectoralis muscle (Figure 2). These three incision sites, however, leave visible scars. Recent developments in endoscopic instrumentation have enabled surgeons to perform many types of surgical procedures through small incisions located at a distance from the surgical site.

The redesign of people-powered portal About.com goes more than skin deep. Luring more advertisers is the next step. About.com has evolved since its inception. The site debuted in February 1997 as the Mining Company, a name that alluded to its roots of mining the Web for information. Since then, the site has transformed itself from a traditional portal service to one populated by real people who personalize–even humanize–he Internet experience. The sites 700 guides cover an eclectic range of topics, from economics and ecology to Shakespeare and soap operas. Unlike search engines or directories, the guides sift through the Web to find the best links, news and content on their topics, providing human judgment and integrity. In addition, they write original, timely and relevant content for their sites. The guides started to write some great stories, says Chris Briggs, creative director for About.com. A lot of guides have gems of knowledge that should be brought to the surface. To reflect the shift in focus and better communicate its topical approach, the company changed its name to About.com in May 1999. However, the name change was not enough. The sites old, drab design, reflective of its portal origins, failed to impart About.coms new content-heavy, one-stop-shop nature. So, seven months later a redesign began. At a time of war and tax cuts and huge deficits, something has to give. Proposed Legislation Would Expand the Chiropractic Benefit Worldwide Dynamic Chiropractic examines House Resolutions 5122 and 5202, two important pieces of legislation that would significantly impact the provision of chiropractic services to U.S. military personnel and veterans around the world. Providing chiropractic benefits to the nations veterans has been a goal of the chiropractic profession for decades. B.J. Palmer first promulgated the idea of making chiropractic a health benefit for veterans as early as 1937, and even succeeded in having a bill introduced in Congress that would have given veterans access to chiropractic services. While Dr. Palmers original efforts were unsuccessful, the profession finally reached its goal in January 2002, when President George W. Bush signed into law the Department of Veterans Affairs Health Care Programs Enhancement Act, which established a permanent chiropractic benefit within the Veterans Affairs health care system.

The power contribution of each cylinder of a diesel engine can be used to assess engine condition and assist in locating faults (i.e., fuel rack adjustment, injector fouling, valve seating, ring breakage, etc). In larger diesel engines, cylinder condition is usually assessed by measuring cylinder pressure during operation using permanently installed cylinder pressure access ports. Smaller high-speed diesel engines, such as the Lister-Petter HL-6, are not normally equipped with combustion pressure access ports and the cost of modifying cylinder heads made this approach unattractive to DND. Advanced Engine Technology Ltd. (AET), has developed an engine condition monitoring system for this application under contract with the North Warning System Office of DND. AET is a research, development and manufacturing company with diesel engine/fuels laboratory facilities located in Nepean, a neighboring city to Ottawa, Ontario, Canada. The AET Engine Condition Monitor (ECM) can detect any combination of cylinder faults in six-cylinder high-speed diesel generator sets under continually varying loads. The power balance of each cylinder can be predicted to within [+ or -]2 percent RMS and cylinder pressure measurements are not required. Operation of the ECM is fully automated, and engine data can be downloaded from the remote radar sites to a centrally located monitoring station.

Overnight it seems that the humble garden shed has been blasted into the realm of philosophical speculation, in the wake of a book by an American, Michael Pollan. So if you feel the urge to raise a structure at the end of your garden, what are the pitfalls to look out for? And can a shed be more than just a place to store the garden tools? Can it really be a retreat for the adults of the house, away from the spouse and the kids? Roger Trapp, a journalist based in Dulwich, south London, turned this idea on its head when he converted the shed in the garden of his house into a playroom for his children. “The kids helped decorate it and they spend lots of time out there, especially in the summer,” he says. “And it cost hardly anything to convert, including putting in a couple of windows.” Others have turned their shed into a gym, or a home office. More sophisticated uses such as these will almost certainly require an electricity supply and possibly a telephone line and running water as well, and these will push the price up. Labour costs will suddenly become an issue and if you want something elaborate, so too might a bill from an architect. A basic shed should, however, be within reach of those with basic DIY skills. One bonus is that it will almost certainly not require planning permission. Contact your local planning department to check, but as a rule of thumb, a new structure that is less than 4m tall and less than 300sqft in area does not usually require consent. However do, as a matter of course, consult your neighbours, especially if the unit may affect any boundary conditions or be joined to a party wall. If they have grounds to object and get in touch with the council once it is erected, you may find yourself forced to take the whole thing back down. Building a shed can be a creative exercise, from choosing materials to deciding on design and the shape of the structure. DIY companies such as BQ have a range of products, aimed mainly at the gardening end of the market. However, its Apex Chalet, which measures 8ft x 6ft and costs pounds 499.99 is sold as “an attractive leisure building . . . making an ideal retreat”. There are companies with altogether more ambitious plans. Courtyard Designs, based in Suckley, Hereford and Worcester, saw a gap in the market for home office rooms and launched its first design a year ago. Its buildings come complete with power and telephone sockets and all the technological aids required to run a modern office. The company eschews the word “shed”. “It can conjure up images of cheapness,” says Ursula Mason, co-owner. Importantly, Courtyard rooms are designed to be secure. They have an iron grille as one of five wall layers which makes breaking in through the walls virtually impossible. The exterior is treated Douglas Fir and the French door locks meet insurance standards. Each also has a proper slate or clay tile roof. A basic 20ftx10ft room with a height of 12ft 6in costs approximately pounds 17,000. Each is made to order, and there is a degree of freedom for the customer to specify individual requirements. The room is delivered in sections, and assembled on site. You could, however, consider building your own shed. For a timber-framed concrete-based structure measuring around 20ftx10ft, you should be able to find a builder who will quote between pounds 12,000 and pounds 15,000 including the design specifications. Make sure you pay careful attention to insulation; if you are going to use it as a home office, you will need to work there at all times of the year. Check with your builder that he can insulate the building to a high SAP rating - the industry standard for insulation. As the quality of style and furnishings goes up, so too does the need for good security. Burglars often break into outside sheds to steal tools as an aid to breaking into the main property. There is also a thriving market in stolen lawn mowers and a decent set of gardening tools will fetch a reasonable price second hand. Needless to say, any burglar is going to be delighted to find a PC in the garden shed. For this reason you must extend your buildings and household contents insurance to cover the shed and what you have in it. If you use it as a home office, you may need to consider obtaining business premises insurance - a high street insurance broker should be able to advise on suitable policies.

The book under review is an expanded English version of Sardellis La musica per flauto di Antonio Vivaldi (Florence: Leo S. Olschki, 2001), translated by one of the most eminent Vivaldi scholars of today, who also contributed some of the additional material. The original book constituted a considerable achievement in woodwind scholarship: the first book-length study of Vivaldis imaginative music for flute and recorder (in Italian, both kinds of flauto) by someone well-versed in the composers music in general as well as previous research on the subject and possessing an obvious love for the period. The main preoccupations of the book were matters of instrumentation, dating, and the players and occasions for which pieces were written. Perhaps the most stimulating section, on Vivaldis use of the flute and recorder in vocal music, explored virtually uncharted territory. The translation retains the books division into two parts, of disparate length (54, 134 pp.). In part I, The Recorder and Flute in Italy in Vivaldis Time, Sardelli looks at the social, musical, and organological evidence for the presence of the instruments, both in the country and in Vivaldis life. This skillfully assembled mass of evidence supports Sardellis novel reversal of the received scholarly view that, rather than writing for the recorder in the first two or three decades of the eighteenth century, then switching over to the flute, Vivaldi already preferred the flute in the 1710s and did not start writing for the recorder until the early 1720s. In part II, on the music, Sardelli devotes chapters to the sonatas, chamber concertos, flute concertos, recorder concertos, concertos for flautino, concerto for two flutes, concertos with multiple soloists and orchestra, and finally vocal music. I was especially taken by his evidence and arguments that Vivaldi himself taught the flute and had an insiders view of the technique of both instruments. The section showing how Vivaldi reworked the C minor recorder concerto from the violin concerto, RV 202, blends musicological and practical considerations in a masterly way. Sardelli has had two particular advantages in his research. The first is a special kind of bibliographical control: my own, still unpublished, catalogue of Vivaldis self-quotations–a massive accumulation. which I hope will appear in print before long (pp. xvi-xvii). The book is living proof of Sardellis opinion that critical analysis of the dense network of borrowings, reworkings and quotations that pervade the entire output of the composer can play its part. in establishing more firmly the chronology and filiation of the sources (p. xvii). Second, Sardelli has had private access to Peter Ryoms forthcoming large and complete version (Grosse Ausgabe) of his monumental catalogue, the Verzeichnis der Werke Antonio Vivaldis, which is soon to appear from Breitkopf Hartel (p. xvii).
Performing Music in the Age of Recording. By Robert Philip. New Haven: Yale University Press, 2004. [vii, 293 p. ISBN 0-300-10246-1. $35.00.] Music examples, index. The basic premise of Philips book is that recordings have not only had an effect on the style of performance but on the actual level of performance standards, and on the expectations of the listener. The first chapter, recounting the life of the musician in the nineteenth century, emphasizes the lack of opportunity to hear not only the great performers of the era but also to hear major musical works as they were intended to be heard. During the nineteenth century and the first part of the twentieth century most people knew the works of Mozart, Beethoven, and others only in arrangements either for keyboard or small ensembles. As interesting as many of these arrangements are, they cannot convey the power and musical interest of a full orchestra performing the same work. The important virtuoso soloists rarely had a chance to hear each other since they were seldom in the same city at the same time. Philip points out that this means that each performer retained their individual style without much influence from other performers. The heart of the book is the discussion of changes in style and performance practice of instrumentalists. By comparing performances by the same conductor with different orchestras and those by one orchestra under different conductors, Philip gives the reader a fascinating look at how great the changes have been in musical performance between the first and second halves of the twentieth century. His comparisons of Arturo Toscaninis performances with the La Scala Orchestra and the NBC Symphony are interesting, but the standards of most opera house orchestras until very recently could never compare with those of a major orchestra, so it is difficult to know how to attribute the differences. It would have been interesting to know his views on the changes that took place in performances with the NBC Symphony of the same work, such as Beethovens Coriolan Overture that was broadcast in 1938, 1944, 1946, 1949, and 1953. The constancy of conductor and orchestra would allow for a different type of comparison and open up new avenues of discussion. This idea is covered briefly in the discussions of the piano performances of Vladimir Horowitz and Artur Rubinstein, but most of the discussions revolve around the differences between the performances of different performers from different eras.
The Best In Recorded Music Chante Moore returns from a long hiatus to deliver a new recording, This Moment is Mine (MCA), and in the process demonstrates that she still has one of the prettiest voices in the music business. Throughout the 14-track set, she hints of personal feelings and experiences. Among the standout tunes is the melancholy “I Cry to Myself,” which has a jazzy, bluesy feel. Then there are the fervent, passionate ballad “In My Life,” the easy-flowing, romantic “Easy,” and the self-assured title song, “This Moment is Mine.” On each of the songs, Moore serenades listeners with easyflowing tempos and beautiful vocals. Walt Whitman and the Soul Children of Chicago showcase their many talents with Generation X Celebration (CGI), a fitting title for an energetic recording that features a choir with members whose ages range from 7 to 17. The gifted Whitman brings out the best in his youth with fine writing and eompositions, and with fresh, energetic arrangements. “My Father Knows,” which features LeCresia Campbell, is a beautiful ballad, while Deitrick and Damita Haddon deliver stirring vocals on the anthem-like “Stop The Tears.” Theres also the jubilant “Power,” and the jazzy “Play On” instrumental interlude, written and performed by Aaron Lindsay. A standout is the timeless “His Eye Is On The Sparrow,” with Keisha MeFarland on vocals. Whitman does an excellent job as lead vocalist, arranger, choir director and mentor to this great cadre of youthful talent.

Health Alliance Medical Plans last month announced its coverage of two over-the-counter drugs formerly available by prescription only: Wyeths Alavert and Procter Gambles Prilosec OTC. Health Alliance will require members who have prescription drug benefits to obtain a written prescription from their physicians for the new over-the-counter drugs in order to receive coverage. The news at Health Alliance follows Medco Health Solutions expansion of its Generics First program in June to include Wyeths analgesic Advil and allergy remedy Alavert.

Firms are facing financial difficulties because they are too slow to grasp new technology, according to Graham Whitehead, principal consultant and futurologist at BT, who spoke at the ITService Management Forum conference last month in Brighton. Whitehead pointed to encyclopedia publisher Britannia as a great example of a firm using new technology and learning to adapt. The company has published a L39 DVD of its complete encyclopedia collection in addition to the hard copy volumes. “Firms are going out of business so quickly, Whitehead added. They have about 10 months to get it right once they start having difficulties.” Geoff McMullen, president of the British Computer Society, agreed that change was becoming increasingly rapid and was multi-dimensional. Half of what we know today will be out of date within three years, he said. But he cautioned firms not to get carried away by technology. You can get the new technology, but your infrastructure must be developed slowly and carefully. People must be trained and the vision communicated within the organisation, or the technology means little.”
Currently, there are so many new technologies emerging so rapidly it can almost be overwhelming. Are computer mainframes going away? Will personal computers and client/server systems take over the computing world? Are telephones, cable TV and personal computers somehow going to combine technologies to deliver new services? What is the Information Super Highway? How will these technologies affect our lives and our businesses? Some new technologies will prove to be viable in the long run; many wont. Being the first out of the gate to associate with a new technology, first to bring it to the marketplace, or even being the company that invents a hot new technology is not the focus of the technology manager.To a technology management company, the challenge is in determining how to capitalize on either new or existing technology to the clients greatest advantage and benefit. A common cry from executives today is, Ok, so all this technology is out there, Now, how do we use it to earn a profit or to improve productivity or customer service or responsiveness? In 1775 Elizabeth, dowager countess of Home, hired the renowned architect Robert Adam to design and complete the interiors of her newly built house at 20 Portman Square, London (Fig. 1). (1) While this patron and commission may not be among the most prestigious of Adams long career, the finished house is widely regarded as one of the best examples of his interior design genius. Lady Home was an immensely rich Jamaican heiress whose fortune increased in 1734 with the death of her first husband, the Honorable James Lawes–the son of the British governor of Jamaica. Lady Home acquired her title through her short-lived second marriage in 1742 to the impoverished spendthrift William (d. 1761), eighth earl of Home, who had abandoned his first wife two years earlier. The new Lady Home became a colorful figure, affectionately known as the Queen of Hell, (2) in the somewhat disreputable social circles of London, and she was a close confidant and supporter of her niece-in-law Anne Horton (1743-1808), duchess of Cumberland. (3) In possession of her magnificent house, Lady Home could show off to the respectable elements of London society as well as provide herself and the Cumberlands with a showcase for their anti-court social set. (4)

A MAN CONSIDERING laser-assisted in-situ keratomileusis (LASIK) surgery, which is done to correct nearsightedness or astigmatism, asked the ophthalmologist about possible complications. The physician told him they occur in rare cases but go away naturally or can be corrected by follow-up “enhancements.” He said the man was an ideal candidate for surgery. Although the LASIK procedure is relatively safe, complications can occur. Having received reports of resulting eye injuries and complications, the FDA looked into the issue and launched a Web site to bring unbiased information to consumers. If your patient is considering LASIK surgery, tell him to take these precautions:
The cornea is the main refracting element of the optical system of the human eye, and small changes in the anterior curvature will produce relatively large changes in the patients overall refraction.(4) The term keratomileusis refers to refractive keratoplasty techniques that combine a lamellar (ie, horizontal, platelike) resection of the cornea performed to change the corneal radius of curvature with a microkeratome (ie, a knife that is used for incising the cornea) or other instrument and cryolathe, microkeratome excision, or excimer laser ablation.(5) Refractive corneal surgery is a cornerstone for invasive vision correction from which subsequent surgical techniques and innovations are evolving. The LASIK procedure uses the excimer laser for photoablation of the corneal stromal bed to alter the corneal curvature, bending admitted light onto the retina and generating a refractive change. The reported benefits of the procedure are rapidly improved visual acuity and minimal postoperative recovery.
